Media Release and Consent for Use of Information

I hereby grant permission to The Neighborhood Beverage Company, Inc. dba Once Upon a Coconut (“OUAC”), its representatives, and employees the right to use my name, likeness, image, voice, and/or statements for the purpose of any and all of its publications, including web-based publications, without any payment or other consideration. This consent includes, but is not limited to, the following uses:

• Social Media: all social media platforms.
• Press Releases: Distribution to media outlets, inclusion on OUAC's website, and other online platforms.
• Marketing and Promotional Materials: Brochures, flyers, posters, newsletters, and other promotional materials.
• Media Publications: Newspapers, magazines, television, radio, and other media platforms.

I understand that the information provided may be used in various media formats including print, digital, and electronic formats. I also understand that I will not be compensated for the use of this information and that this consent is granted without any expectation of payment or other remuneration.

Duration of Consent:

This consent is given in perpetuity unless I revoke it in writing. I understand that I may withdraw my consent at any time by contacting OUAC at [email protected]. Such withdrawal will not affect the information and materials that have already been published prior to the receipt of my written notice of withdrawal.

Acknowledgment and Understanding:

I HEREBY HOLD HARMLESS, RELEASE, AND FOREVER DISCHARGE THE OUAC FROM ALL CLAIMS, DEMANDS, AND CAUSES OF ACTION WHICH I, MY HEIRS, REPRESENTATIVES, EXECUTORS, ADMINISTRATORS, OR ANY OTHER PERSONS ACTING ON MY BEHALF OR ON BEHALF OF MY ESTATE HAVE OR MAY HAVE BY REASON OF THIS AUTHORIZATION. I ACKNOWLEDGE THAT I HAVE READ AND FULLY UNDERSTAND THE CONTENTS OF THIS CONSENT FORM. I AFFIRM THAT I AM OVER THE AGE OF 18 OR HAVE OBTAINED PARENTAL/GUARDIAN CONSENT FOR THIS AGREEMENT. I UNDERSTAND THAT BY SIGNING THIS CONSENT FORM, I AM WAIVING ANY RIGHT TO INSPECT OR APPROVE THE FINISHED PRODUCT OR ANY USE OF THE MATERIALS.
